     The most common skin problems that presents with recurrent or chronic itch or rash on face, trunk and limbs including Allergic contact dermatitis, Seborrheic Dermatitis, Atopic dermatitis and Eczema are often treated with topical/systemic steroid medicationsanti-allergy pills, anti-inflammatory drugs, immunosuppressant or anti-biotic depending upon type and severity of the disease. Therefore, the medications should be prescribed and supervised under dermatologist control. Treatment may give quick results but sometimes it may take longer time. Some people may require long term treatment with topical or systemic immunosuppressant for a period of several years to control the symptoms.

     Dermatitis refers to inflammation of the skin which is one of the most skin problems found in dermatological practice. It can be classified by etiology as below.

  • Exogenous or Contact Dermatitis such as the exposure to irritants or chemicals and cosmetics which cause allergies.

  • Endogenous or Constitutional Eczema is caused by changes in the body or hereditary allergies.

Exogenous or Contact Dermatitis

The skin reaction that caused by direct skin exposure to chemicals. The most common types are;

 

Irritant Contact Dermatitis

Irritant Contact Dermatitis is the skin dermatitis and irritation due to chemical exposure during daily life such as doing housework, scope of work by occupation and environment. The substances that often cause the symptoms are detergent, highly concentrated acid-base and etc. Symptoms can occur to anyone and occur even on only the first exposure. It usually begins with itching, redness or burning sensation on skin. In extreme cases, after discontinuation of substance, the symptoms will subside but some may have continuous allergy symptoms.

 

Allergic Contact Dermatitis

     Allergic Contact Dermatitis is the skin dermatitis caused by the exposure to substances that causes allergies. It is a symptom that occurs with only certain people. It is an itchy dermatitis.  Substances that frequently cause allergic reaction are nickel metal, rubber gloves, shoe making materials, including various cosmetics such as dye, fragrances and preservative etc.  Symptoms usually occur after second exposure or subsequent. Stop using substances cannot make rash disappear. The symptoms usually persist for 1-2 weeks. Sensitization may occur again when exposed to such substance again.

Patients who are allergic to cosmetics, mostly found in people who use unstandardized whitening cream/bleaching cream.

Diagnosis dermatologist will examine the history and physical examination to diagnose substances that can cause allergic reactions.  Then the doctor will do Patch test to confirm.

What is Patch test?

Patch test is an allergic rash test resulting from exposure. The substances that commonly cause allergic rash are rubber gloves, nickel metal, hair dyes and chemicals used in dyeing, preservatives, fragrances and etc. This test aims at allergies confirmation and suspected exposure rash position such as nickel metal allergy which usually has earring allergic rash at earlobe. Preservative allergy mixed in sunscreen products and all types of skin cream even Some sunscreen allergy such as Oxybenzone or cosmetics perfume allergy  which rash usually appear on facial skin.  Allergies can be caused by more than one substance. Doing Patch test can test more than one suspect substance at once. This is to identify substances that truly cause symptoms.

Patch test

 

  • Bring product in use or suspected cause of allergic reactions such as ointment creams or cosmetics together with package and label.
  • Put plaster containing substances that are needed to be tested on your back and leave it for 48 hours. After that, doctors will appoint for the first reading and will have the second appointment after 96 hours.  During such period, be careful not to wet your skin. Typically the plaster is put on your back but sometimes on the forearm in case of fewer substances.
  • Avoid activities that sweating a lot while testing such as sport.
  • This test cannot be performed in pregnant women.
  • In the case of steroids, both tablets and injection drugs should be avoided for 1-2 weeks before the test, because the steroids have immunosuppressive effect that causes the error.

Endogenous or Constitutional Eczema

Caused from body transformation that may concern genetic. There are many kinds of diseases in this group. The common types are Atopic Dermatitis and Seborrheic dermatilis.

Atopic  Dermatitis (AD) is a skin disease associated with asthma, pollen allergy. It is characterized hereditary with patient's level Immunogiobulin E high (substances found in people with allergies). Most common symptoms are found in childhood, only a minority of those found in other age ranges. The AD patient’s symptoms in general are dry skin and less sweat.   In children, the symptoms are dry, flaky skin on the 2 cheeks.  When grown-up, it may spread to the neck or the outer sleeves. In children about 2 years or in teenager to young adult, the rash is usually found in the crook of limbs, face and neck. Usually, symptoms will subside over time when subjected to eight years.  But will come back again when entering a young age.

Promoting factors that cause AD disease

  • climate change such as dry or hot
  • Wearing clothes made from synthetic fibers or fur.
  • Using soap or ointment that cause irritation
  • Environmental pollution such as in an atmosphere filled with dust or pollen, tobacco smoke.
  • Stress and anxiety
  • Using soap for sensitive skin or easily irritated
  • Condition of excessive sun, wind, sea, sand.

Seborrheic dermatitis (SD)

is a symptom of skin disease which cause erythema or oily clear edge with dry flakes on the top and white-yellow flaky that often peel off.  Three areas that often experience symptoms are;

1.  On face, nose, between the eyebrows, behind the ears or scaly in the ears neck and may have eye inflammation.

 2.  Above the ribs, upper back and chest, armpits, if it occurs on the scalp, it is called dandruff.

3.  In infancy it is found in the groin area, under the chest and navel.

People with Seborrheic dermatitis may produce oil which contains too much acid causing skin inflammation or because of yeast named Malassezia grown up in the sebaceous glands along with the bacteria which cause more durable inflammation.

Seborrheic dermatitis (SD) is typically found in three age ranges including infant after birth, youngsters and elderly because the symptoms of SD is similar to Psoriasis symptoms. Diagnosis therefore requires special expertise from medical specialist. The symptoms of SD are also associated with other diseases such as Parkinson, neurological and brain diseases and Malassezia infection

Prevention and treatment

Scalp Seborrheic Dermatitis

Use a shampoo containing either chemicals approved by FDA which are salicylic acid, coal tar, zinc pyrithlone, selenium sulfide, sulfur, azole and ketoconazole.People with such symptom may try many shampoosto find the type that suits their own condition. Switch your drugshampoo with generic shampoo or with other drugshampoo.
